Conditional Waiver and Release on Progress Payment

A.R.S. § 33-1008(D)(1)

A.R.S. § 33-1008(D)(1) sets out this form for a waiver and release given in exchange for, or to induce, a progress payment where the claimant is not in fact paid in exchange for it, or where a single payee or joint payee check is given in exchange for it.

Unconditional Waiver and Release on Progress Payment

A.R.S. § 33-1008(D)(2)

A.R.S. § 33-1008(D)(2) sets out this form for a waiver and release given in exchange for, or to induce, a progress payment where the claimant asserts in it that it has been paid the progress payment. The statute requires each unconditional waiver to contain the notice in type at least as large as the largest type otherwise on the document.

A.R.S. § 33-1008(D)(3)

A.R.S. § 33-1008(D)(3) sets out this form for a waiver and release given in exchange for, or to induce, a final payment where the claimant is not paid in exchange for it, or where a single payee or joint payee check is given in exchange for it.

Unconditional Waiver and Release on Final Payment

A.R.S. § 33-1008(D)(4)

A.R.S. § 33-1008(D)(4) sets out this form for a waiver and release given in exchange for, or to induce, a final payment where the claimant asserts in it that it has been paid the final payment. The statute requires each unconditional waiver to contain the notice in type at least as large as the largest type otherwise on the document.
